HERE IT IS! From Tmobile and passed along to you... it may be different...so if it is, sorry. Dont try to do it more than 3 times if it doesn't work. If it doesn't work, you'll have to call the provider its locked to.

NOTE: If you receive an error message while performing the instructions below, stop. Please be aware that if an excessive amount of incorrect attempts are made, the phone will be permanently locked. Verify the IMEI by pressing *#06# and submit a new request.

1. Enter #, 0, 1, 1, 1, *, [unlock code], #
2. Press the SEND key and the display should read, "Personalization Cancelled"

NOTE: "Not Yet Personalized" will be displayed if it has already been unlocked

i remember when I had tmobil I locked my phone and tryed to unlock it till it the sim cars locked itself, well I had to buy a new sim card!
