What plugs into the PCV?

What plugs into the PCV? I am changing my intake manifold right now and the PCV is a Y connection. One end of the split plugs into the front of the manifold, what does the other part of the split plug into and what does the other end plug into? I can't remember from from I took it apart (4 weeks ago).
