Default SES light - code P0405

Service engine soon light went on, checked the code with OBDII scanner (free rental from Autozone) and it displayed:
Code P0405
EGR Flow Sensor A Circuit Low Input

No other symptoms. Light is on when engine is running almost always, sometimes doesn't go on when the engine is started hot.

Does anyone know what this means/how to fix it?
