Belt Tensioner problem after UD Pulley?

Default Belt Tensioner problem after UD Pulley?

My car has tossed a belt twice now...basically comes outta line and almost off the power steering pulley. Never did this before the cam and UD pulley.

The first time it happened was when it was on the dyno. The second time last night after doin some good driving prior to an oil change. It was off line, then I started it forgetting about the belt off line, then it flew off.

Do I need a new tensioner??? Or just a new stock one....I shift at 6200 RPM's.

BTW, I have a new belt on that went on with the UD Pulley.

could be tensioner or alignment.
