Default Spark Plug breaks

I noticed I had a misfire and decided to change my plugs. Last night i was changing my spark plugs and found that the one on the driver side on the far back of engine broke the ceramic white peice. When i saw the busted up spark plug, I knew it had been the problem all along. After changing all of the plugs, the engine had no more misfire. I was just wondering what would cause the ceramic to break off like that? It had AC Delco Rapid Fire plugs #14...just wondering...dont want it to happen again...

It usually happens on installation or removal. I suppose vibration could cause it if it was weak/defective but thats kind of unlikely.
