Who has had a clutch or pressure plate come apart?

Who has had a clutch or pressure plate come apart? I know I've seen posts about this before, but I can't seem to find any of them when I search. Maybe I'm not searching the right terms? I'd like to know three things.
1. Who has had a clutch disk or pressure plate come apart (explode) during operation?
2. What brand/level/style was the clutch/pressure plate?
3. What were the conditions in which it came apart (ie. 6000K launch....)?
Just trying to determine if any one brand of clutch is more prone to this type of failure. Thanks guys.
