Default PCM is not giving command to shift to OD

PCM is not telling transmission to shift to overdrive (according to transmission builder). What does the PCM need to see in order to shift to overdrive.

Default

There are 2 or 3 tables that tell the trans when to shift into 4th. Most are based on thottle position and mph. There is also a rpm-based shift table for WOT only.

The PCM should throw codes if there's a bad solenoid or something in the trans. Not sure if there is some form of "limp mode" that prevents shifting into 4th.

Are you experiencing this problem in normal driving? Have you used any kind of scan tool to verify what gear the PCM is commanding?
