I was wondering this too...

A search turned up a couple of methods:

The majority of the posts say:
Who to get a hold of from ASC for your Conversion#:
Email: marty.straub@ascglobal.com or
Make sure you include your VIN# as well as your Year.

*Please be patient when waiting for a response from Marty. He is taking his own time looking up your info and then emailing it back to you.
I saw many posts that said you wouldn't hear back from Marty until 4-6 months later, so I emailed him today.

I also found this:
the built # comes from ASC. you can call at 743-246-2772 and ask for a nancy barnett, or email Marketing@ascglobal.com.
I didn't try this, if this works quicker for anyone let me know!

ASC # and conversion date: emailed.... justin.fabiola@***.net
Justin has access to the information needed and is great to work with.
BACK TO MY ORIGINAL QUESTION........Does anyone know where I can get a dash plaque to display this info?

I would go to a local trophy shop for baseball and stuff they could probably make what u want.

Also I talked to marty and he gave me all the info you just have to be patient he does it on his own just as a favor for us.
