Default th400 random slip in second

Well I have a th400 built by Coan with a RMVB and transbrake. After not driving the car for awhile (few weeks) I took it out today only to be disappointed to have 2nd gear slip 4 times on me randomly. Three times while just driving normally and shifting into second and once getting on it and into second. Seems like after i come to a stop and shift it back into 1st then up thru again the problem goes away. I have about 400 miles on this setup if that. The converter is a brand new "W" series converter from FTI. The transmission was used for this season then pulled to go to a glide.

Car is a forged 370 with a TC78 turbo currently only seeing 5lbs of boost.

A slip in 2nd would lead to a pressure issue, worn intermediate clutch or a damaged intermediate sprag.
