6x issues

6x issues Hey guys, I've had my 6x installed in my LS swapped Bmw for a year or two. I drove the car about 4-5000 miles when it went out, reverse failed and wouldn't shift right. So I sent it in for repair, they said it got hot, had it installed with a mocal cooler, which is enormous in comparison to the tci cooler.
Now, 1500 miles later its out again. Drove it to work one day and got in to go home... No gears at all, didn't even try to engage.
So now it's out of the car AGAIN! And I took a couple pics of the damage the shop pointed out to me when they pulled it.
Brass in the pan
Input shaft/front pump bearing (note the hole in the bottom left corner)
So it's going in for repair again, but I'd like to know how this happened.
