Default 4L60e swap takes off in 2ND
I did a LQ4 swap with 95 4L60E and 0411 PCM. The trans starts in 2nd gear, but when I pull it in 1st manually it will go in 1st, and when shifted it will go into 2nd. I was told it may be the PWM, not letting it drop into 1st gear. Have anyone had this problem.

No transmission codes, but I do have MAF and #7 cyl misfire

Sounds like it is in limp mode. Fix your MAF and misfire issues asap and that should get you out of limp mode.
