Hughes Adapter with TH350 issues
I would not recommend running it less than a 1/8" pilot engagment

Can you post a picture of the back of the converter?
Also Its hard to tell from the photo you have but looks like the pilot extension may be backwards? The small side should engage into the crank while the larger side should slide over the pilot of the converter.

Yeah from my view too look like its on backwards
Edit: It also looks like in the way your saying is your putting the pilot extension on the crank and then trying to slide the flex-plate over it. I could be wrong and be misinterpreting what you said. But if that the case your are using the pilot extension the wrong way.

And no, I slide the pilot adapter small side into the crank. It doesn't interfere with the flexplate.
Not my picture but this is the same way I am using mine.
No. What I did was slide the hughes adapter into the crank. Then I bolted the converter to the flexplate. When I slide the flexplate with converter onto the crank the pilot of the converter bottoms out on the adapter and keeps the flexplate from seating.
OK gotcha so the Pilot extension is not the issue its that the converter pilot is too long and bottoming out on the extension. Were you able to measure the converter pilot and how far it sticks out?
No I forgot to get that measurement last night. I will tonight.
That converter cover is available with different height pilots(screw on). Take a photo looking across the pads of the converter and the pilot. MY bet Is they have the converter pilot setup for 1/4" mid-plate or something similar.
