Default 4l60e stock possible issues

I have a 2001 5.3/4l60e stock swap in my blazer with a stock tune and ecu.

Long story short my 3-4 gear shift is odd, I can allow the engine to get to operating temp and take the truck out and it will likely shift into OD and I can hear the lockup right before but when I've driven it a few miles it acts like it doesn't want to hit OD. If I am cruising at 50 or so and give it 1/4 throttle or so you can feel it hit into OD and primarily stay there until i slow down below 45, sometimes it will wind up 3rd gear longer than others with generally the same amount of throttle applied.

I know this doesn't really help much but if its very warm and I apply very light throttle in 3rd it just doesn't seem like it wants to hit 4th by itself until I give it a little more throttle, also you can hear it jump out of 4th and back into 3rd a lot unless its a very constant speed in 4th.

If anyone has ANY suggestions please feel free to give me some input, the trans filter is probably original (95k) but this came from a known running driving and shifting truck. Could this be tuning, low fluid? I dont have the correct dipstick so I couldn't tell the actual fluid level, but I added 2 qts of fluid and it seemed much better because originally it didn't want to stay in 4th at ALL.

Confirming the correct fluid level is the absolute first step.
Next step is to connect a scanner, check for (hidden) codes and make sure the TPS and MAF are working correctly.

After that, I would suggest taking it to a shop with a scanner that can command the different gears and evaluate how it is shifting. A good trans shop would also connect a pressure gauge to watch the line pressure under different throttle settings.

If you want to tackle this yourself, we can give you more info on these steps.

Oddly enough I am using a TH350 Dipstick tube which reads differently but I am fairly confident the level is correct at this point but I do not want to overfill it.

I have an Actron bluetooth scanner that doesn't seem to be showing any active codes but I am unsure if it actually does Trans codes. I previously check the MAF and TPS readings on another one of my posts for a completely unrelated problem and everything appeared to be reading correctly but I can verify these with you.

I actually have a trans pressure gauge I bought years back for another project but the line isn't nearly long enough to bring up through the cab while driving.
