Default 4L80E fluid leak

So I have been through the wringer with transmissions in my van and need some advise as to why a pump bushing would tear up. Trans was working perfect for about a week and suddenly started dumping the fluid out of the pump. Pulled it and the converter snout looks scored along with the pump bushing. Seal was literally pushed out of the pump (assuming the fluid pressure caused that). My question is, is the bushing being messed up typically due to the converter or ?

maybe just a bad bushing? Flexplate all tight no play to make the converter move around?

Flex plate is good. I was thinking the torque converter had some scouring when it was put in but ignored maybe. I don't remeber seeing or if I even paid attention to it when I pulled the cover off the snout and threw it in. Just looking for some secondary advice or reassurance before swapping the pump and new converter.

Converter hub should be smooth through out. Looks like it has some chattering going on around the pump gears and possible bushing scoring. Can you feel a groove in the hub where the bushing rides?

Is the converter neck out of round?? Is the converter pilot sitting in the flywheel/crank correctly?
The bushing might be egg shaped in the pump. Put the pump half on the converter neck and see how it feels. Something is out of round!!

Installed new pump bushing and went cheap Transtar torque converter with billet pump or hub and it's been running perfect. Temps stay below 150* and just got word from very well known converter company that the hub was on an angle causing an out of round effect and reason for bushing.
