Default Burnt clutch?

What keeps causing me to burn the clutch in my converters? I had it for only 5 months and haven't raced with it yet. Pulled it to get restalled and precision told me the clutch was burnt out! WTF, thats twice were I had a converter that seemed fine burn out the clutch. Is it in the tuning?

Your transmission pump or valvebody can have a internal leak
this will wear out the clutch early
