Non-LS1 Transmission Problem... Need suggestions

Default Non-LS1 Transmission Problem... Need suggestions

2000 Ford AX4N 4-speed automatic trans
Ford Taurus w/ 3.0L Vulcan

Need some help with this one. Please take a look at the description of the problem below and let me know what you think. Is it dead? Anything specific to check?

Drive #1
(Cold) Started and drove approximately 5 minutes, without problem. Sut off for approx 5 minutes and restarted. Upon pulling out, approx 1/2 throttle, the trans would not shift into 2nd. Upon backing off on the throttle, trans shifted into 2nd. Started testing... light throttle, trans would shift ok, half-throttle or higher trans failed to shift untill rpms got pretty high or throttle was backed off. When it did shift in the higher rpms, it whould shift pretty hard. Returned home, checked fluid level. fluid low. Added trans fluid to midpoint on stick. Vehicle not driven until following day.

Drive #2
(Cold) Again ran fine for approx five minutes without problem. Shut down and restarted with 5 min wait. Complete loss of 2nd gear or higher, regardless of throttle. Trans would not shift out of 1st gear. Car let sit overnight.

Drive #3
(cold) Started and driven back home, approximately 7 minutes travel time, no problem.

It seems like the problem is temperature related, as there is no problem during cold starts. Once everything has been warmed up, the problem starts. In the past, under light throttle, the vehicle would sometimes sometimes stumble or have a slight shudder which went away when additional throttle was applied or removed. Occasional hard shift under part throttle.

Any ideas on what this may be, or am I screwed?
