Default Vortec Engine Cover (top/cover/lid/what-do-you-call-it) Question...

I bought and now have installed a 5.3 in my vehicle. Anyhow, the engine came without the top/cover/lid (heck, I don't know what you guys call the thing). Do you guys know:

1.) What's it called?
2.) How does it bolt to the engine?
3.) What's the going price for this thing and it's mounting hardware?

You need to have a truck intake which has two pedestals built in for the cover. None of the "car" intakes have these mounts.
There are two basic designs (and a several logos) that I'm aware of which depend on year and model the engine came from. I believe the newer design is the rounded look you posted and the rectangular one is the early design but I believe they are interchangeable.

Thats the same cover that came on my 2003 GMC 1500HD with 6.0/auto. It slides into two clips in the back and has one bolt towards the front center. I can take pictures if you would like, let me know.
