I just did this swap and i used a 6.0 flex plate/spacer/longer bolts. My converter was drilled for 2 bolt patterns and it lined up perfectly. Im using the stock truck starter. Im not using a kick down so i have no idea about that question

I used a Lokar TPI kickdown cable available at Jegs and works perfect.
