PVC Part numbers?

PVC Part numbers? Looking to install a closed PVC system on a LY6 with LS3 intake, Need some help with partnumbers and confirmation on routing.
Think I need to go from the Aft L/H valve cover (.400" or ~3/8" hose) to the R/H side of the intake(.362 or ~5/16" hose), need either a PVC valve that has the proper hose sizes on each end or some type of PVC Boot that would adapt to hose.
The only other PVC port is the R/H foward valve cover that I will plan to run to the intake between MAF and TB. Correct?
Only other inatke port is the LH side(.310") just aft TB I assume I would cap off.
