I hate to say it, but it was asked if you check the plugs after any of your pulls- Just because you have the fuel system, doesnt mean its working- Ive tuned cars and had them at perfect AFR- but could tell they werent running right- simple plug pull told the story. - Still think its fuel or cam

We pulled the turbo off and it still does it. Just for the heck of it I am going to swap the PCM since that is pretty easy but like others I am leaning towards the cam being a tooth off or even just replacing the cam.

Have you measured your backpressure? I am just throwing ideas out... Big engine.. smaller turbo. Could 5krpm be where you are exceeding your backpressure limit? Whats your hot and cold side piping? I see that you thought of this and went to a .96... but maybe the limit is your pipes not your turbo?
