WTF!! belt broke off!

WTF!! belt broke off! i was out tonight driving around with some buddies, so they tell me to punch it so i do.
the car didn't pull that hard, when looked at the boost guage it was reading 3 inches of vaccuum!!??
the belt broke off after the tensioner pully on the ati bracket loosened up and jamed onto the bracket!! i just tightened the belt and tensioner just before i left that evening!!!??
anyone ever had this problem??
what did you do to stop it from happening again??
