Stalling in reverse kind of?

Probably a noob question but my car starts and idles fine with no problems whatsoever. But when I put the car in reverse and back out the driveway as soon as I come to a stop to put it back into drive the car acts super weird and will completely stall out sometimes. It doesn't always do it though, most of the time I can catch it before it shuts off. In drive it never has any issues. What would that be? Tune? Cam? Iac? It's also finicky and sometimes while normal driving it, say for example everything is fine for 30 miles, but then I'll come to a stop light and the car will surge and almost completely die out again. Any ideas?
Edit: also notice while holding throttle steady at 2k rpm for example the car rpms will drop will holding throttle and air fuel ratio goes way lean into the 20's and acts up.

I think you might have better luck with your question in the PCM & Tuning forum. Your initial description would lead me to believe that your idle tune is off with your cam. But the car going progressively lean while holding steady throttle sounds like a serious vacuum leak.
