Random surge and stall - Help!

Random surge and stall - Help! My wife's truck will randomly surge at a red light, causing the truck to lurch forward which scares the crap out of her. It also sometimes randomly stalls out at lights when going to accelerate. It never has a check engine light and it never happens when I'm driving it.
Its a L76 engine with and ported t/b, airaid intake, catback, ftl stage 5 trans, vig 3600 stall, and prolly a couple other things but its almost stock.
What could be causing this?! I'm at a loss. Thanks for the help.

I'll clean up the MAF but it doesn't have a k/n.
My wife said it stalled out, then when she restarted it is when it started surging. This is the second time it happened in two months!? weird.
