Throwing serpentine belt

Throwing serpentine belt I put on a new gatorback serpentine belt probably 2-3 months ago and now the belt has come off 2 times. The first time was 1 week ago, and it came off when I was backing out of my driveway. The second time just happened today, and it came off under WOT/high rpm.
Should I get the Katech belt tensioner? For those that have had this problem, what fixed it and what didn't fix it?

What he ^^^ said, or stock spring tensioner could be going bad and not holding proper tension - in which case you have the choice of going with solid tensioner such as Katech or TR version or new stock tensioner.
As far as misalignment goes - first culprit I would check is the PS pulley - it only has press fit on pump shaft with no other mechanical holding method.
I have a TurnOne PS pulley and Katech solid tensioner. Just recently I had a problem with the PS pulley slightly walking off the pump shaft. I threw the belt during a 'highway sprint' with a hemi Charger
(luckily it actually just sat contained in the front of the engine and did not get caught up/destroyed).
I re-installed the belt and as soon as I turned the key - belt flew off again!?
Took me a while to see that the PS pulley had walked off the shaft by about 1/4" - throwing off the alignment. I pressed the pulley back on to stop and installed a 3/8-16UNC bolt with washer and Loctite into the end of the pump shaft to retain pulley should this ever happen again.
Other than this problem regarding PS pulley, the solid tensioner has never failed me.

My stock tensioner had too much movement in it. I put the Katech unit on today, nice piece. The pulley has a higher wall, and I can set the tension however I like
