If it is throwing both high and low input malfunctions at the same time what could the problem be?

Are you running a K/N? Have you tried to clean it? MAF

Spray MAF Cleaner (I forget the manufactor).. you should be able to pickup at any parts start or chain...

The K&N filter oil has been know to be applied to heavy and then get a film of K&N oil on the inter of the MAF, whcih causes problems..

When cleaning your K&N Filter, only apply a light coat, just recharge it a bit sooner. but keep the oil applied lightly, only as much as needed. Way to many people just spray the oil on till it is almost dripping wet.. don't do this !

^^^ What he said. I will also take a rag and wipe over the air filter 2-3 times to get any excess oil off.

I have a K&N air filter. All I usually do is take the air compressor and spray all the dirt and dust out of it and put it back in. Never had a problem. I have recently taken it apart to polish the outside of it. It's put back together perfectly. Also added a new ported intake... I'm replacing my 02 sensors tomorrow cause it's throwing both bank 1 & 2 codes.

I would first clean the maf with CRC maf cleaner or if you can't find that then use a electrical contact cleaner. Then clear with code and go for a drive. You will notice a big difference in power if the cleaning work. If it didn't then you gonna have to get a new maf.
