Long story short, These are some Hookers that are pretty old. I noticed a weird sound coming from under my hood so I checked, and fire was shooting out towards my motor. I took off the Headers and here's what I saw. How in the hell do you think this happened?

Is the edge of the opening rusty?

Did the weird start very suddenly or did it build up from normal over time?

No rust. I really don't know. Strange thing, is it's the exact same size as the "Hooker" emblem on the other header, but the hole is on the inside.

Holes in header primaries are usually caused by emissions fittings that fatigue and fall off...if there was an emission fitting there (AIR or EGR), then that's what caused it. Otherwise, I have no idea.

might of been to lean

also could of been a weak spot in the metal and just over time after the pipes heating up and cooling off probably had a lot of wear on that one spot and it prob started as a little hole but as air passed through it got bigger

Probably a weak spot in metal or the emblem is conducting or retaining heat in that area.

When we tried pulling out the driver's side header, we cut off the collector at the end. As soon as the collector was off, one of the primaries actually completely fell off of the header. Lol

This, that was the spot for the EGR tube most likely. Very common failure point.

Dude with all the heat lately around here I would bet it had something to do with it, My rotors warped recently because of all the heat with actually a chunk of metal came off when pulling them.

Im going with the idea that the hooker emblem fell off and took some tubing with it due to the weld cracking along its outside edge.

Header companies usually label BOTH headers in a set, so i highly doubt an EGR port fell off and left a hooker tag sized hole

+1. This happened to my 11-year old Grotyohanns earlier this year. It cost $265 to have them cleaned up, the old emissions fittings closed off, had some welds strengthened, re-coated, and back on they went.

Some of these answers in this thread are plain silly

My air fitting was cracked (Hedman LTs) when I bought the car, then it started to open up further so I cut it off and welded a piece of sheet metal over the hole. Never had another problem out of those headers after that.
