O2 Sensor Failure?

Is it possible for an O2 Upstream sensor to go bad without throwing a code? I just installed the BBK LT headers system, and Corvette O2's (SMP Brand) and am having problems with the car. I got a system lean code on bank 1 and a random/multiple misfire code. I have HP tuners, and scanned the car and noticed that the bank 1 O2 sensor wouldn't go past maybe .150V, even when I accelerated. The bank 2 sensor responds normally and the volts do go up.
Can this be the Bank 1 sensor that is bad? These two are brand new sensors.

Coming from the German word it isn't uncommon for an O2 sensor to go bad without throwing a code. I had two sensors do this to me on my Audi. Switching the banks is the best idea. I also had a MAF sensor do this to me on the Audi. No codes, ran pretty good but when I logged it it would show somewhat funky readings. New maf and the car was back to perfect.

I did find an exhaust leak at the back of the Y-pipe where it meets the back pipe. I'm going to try one of the original O2 sensors(A Denso) that I extended, and see if that works and makes things better.
