Stock manifold bolts to ARP header studs

I'm installing my Kooks LTs with ARP header studs and have a few questions:
1. When I put the studs in, what should I apply to the threads? I was thinking ARP moly lube, anti-seize, or thread locker (like on the stock manifold bolts).
2. When I install the studs, how tight do the actual studs themselves get tightened to?
3. When I tighten the nuts, should I just follow the procedure for the stock manifold bolts - 11 lb ft first pass, inside out, then 18 lb ft final pass, inside out?

Id use antiseize, and do like you said, if you have specs go with them, I just do tight enough to where im not manhandling and breaking off bolts, heat cycle(drive it around a lil bit) and tighten them down again...
