Serpantine belt came off

Serpantine belt came off
I have a off road truck and at high rpm's and hitting kind of hard while off roading, my serpeantine belt will sometimes walk a rib over. It even came all the way off once. The pulleys all look to be in alignment and the tensioner is between the marks. Is the tensioner just worn out enough and cant handle the off road abuse and its time to put a new one on? Any thoughts?
Thanks..........
Thanks..........

I bet something is mis-aligned. I though everything was fine on mine to, but the balancer was the wrong one and it was sticking out further. At very high rpm's the belt would walk to the inside of the balancer and eventually rip itself apart. The only way I eventually found out the problem was to go with a belt one rib less than the original. It would be on the outside of the top pulleys, but to the inside of the balancer. This helped me find out where the problems was.

the tensioners on f-bodies have a rep for going bad, mine was visually crooked and i could easily deflect it with my hand. which accesory setup do you use? the katech tensioner is a nice piece, i am not sure if it would work with truck accessories.
