Need belt #'s

Need belt #'s I just got my ATI Super Damper today. I didn't see anywhere where it says the correct belts to get. Does anybody have the part # of the belts I need? I could also use the Part # for the new crank bolt.
Plus, I have a pulley puller, and a torque wrench. Any other tools I'm going to need for the install?
