Default Belt Fell Off

On Saturday morning I went out and started the Z and had no power steering. Popped the hood and my belt had fallen off. I picked it up off the ground and it had no wear signs anywhere on it and still looks as new as the day I bought it along with my ASP Pulley. (~4 months old) I put it back on the Z and have been driving hard on it for 100+ miles and the car runs fine. Weird I checked the new bolt that was installed with my pulley and its tight. Any suggestions?

BE SURE that the pulley hasn't moved and is not on its way off the crank end. Also, check tensioner for running true. WJ.
