Default Pushrod ends worn off?? has this happened.

Ok, im lookin at a 2000 LS1 engine. that was replaced due to 2 pushrod ends worn off.... has this ever happened or what whould be the root cause of this? they are OEM original pushrods but theres not a bad batch or anything is there?

Bad lifter not pumping oil? Some debris in there got in the cups and chewed them out? hard to tell without looking at the engine/parts.

either way, id replace all the lifters, pushrods and have the pathways cleared at a shop to be safe.

Nope.. pretty sure its the hollow rod rattle... ?? what..
