What will cause a bad lifter?

during a rebuild, not cleaning the cosmolene off or soaking them in oil before installation. too much pressure from the rocker...lifter bore clearance...
on a stock car from the factory...oil starvation/contamination maybe only...i would guess they are pretty reliable unlike the factory rockers.
