Will I have clearance issues?

I ordered a custom comp cam 232 236 595 601 115 and my heads are milled 20 thousandths

Will I clear?

I would say there is a good chance that PTV clearance would be an issue. Either way, I would be sure to check it. It is good practice with any large cam install.
