what is the largest size cam that I can run with 5.3 heads milled down to .013 without having to cut pistons?

But a low range 23x cam can fit with proper valve events.

You may be able to fit something on the order of an f-13, even with some larger valves, although it depends on how far down the valves are hung. Youd need to see how much free valve drop these heads lose vs. stock. If I recall, the f-13 has been run with a .030 or larger mill before, although its cutting things close...
