Default could this be a collapsed lifter??

i just finished the motor (6.0) and i have about 150 miles on it.
the problem is everytime i get the engine warm, the valvetrain ticks ( more like clanks) very loud. i have adjustable rockers and i have adjusted them several times ( at 1/2 turn past 0 preload).
i understand that they are noisy, i had them (rockers) on the old motor but they were nowhere as loud as the are now.

the noise comes and goes but is there while crusing down a highway ,it sounds like a very bad exaust leak or a cracked header .
the scan tool shows that #4 cylinder is showing some misfires (about 60) when the otheres (cylinders) aren't.

could this be a collapsed lifter?? i just put these stock replacement ones in. should i try a different preload on the rockers??
