Default can you install a cam off even thoe it dot to dot

well im thinking in my head about a cam swap.
the valves are all held up but the springs. whern you turn the crank to get the dot up and put the cam in and line it up how do you know what piston is up or down? does any one know what im thinking about? how do you know what piston is up when the the dot is up? or is the same piston up the same time as the dot.

When the dot is up the same piston will be up. Each 360 degree turn puts the piston in the same place it was the last 360 degrees.

yes same with the cam. Just line up the crank dot with the cam dot and youre good to go.
