Had the same issue once with an LS swap. Turned out for me, that I had a harness with the MAP sensor wires flipped around at the sensor plug. Bought it that way. I fiddled with it for a week and then had a tuner come to the shop and put a laptop on it with HPTuners. He found after about 20 minutes of trying stuff that he had no MAP signal. He left and I started ohming wires for the MAP circuit, only then to find the mis-pinning. I flipped 2 wires around and it fired up immediately. Not saying that’s for certain your issue, but after you’ve tried everything that makes sense, look at the improbables. I know yours isn’t a “swap” but I’ve heard of the Lingerfelter box crapping out before.

You said it is a Gen 4 engine in a Gen 3 truck... What computer are you running and if it is the stock Gen 3 computer, did you change the reluctor wheel on the crank to a 24x? Or is the 58x still on the crank?

I see you mentioned lingenfelter box now after going back through.. I assume you mean the 58 to 24x signal conversion box? I would still look there and make sure everything is good with that.
