Default Comeback- Failed AFM lifter

Hello,
I have a 2017 6.2 truck in the shop. Last year, the number 4 intake lifter came apart and bent the pushrod. So I did a soft AFM delete- VLOM modification and oil port blockoff with PCM tuning. I also replaced all the lifters on the right side with new OEM lifters and installed thick wall pushrods all the way around. Everything was great until 15K miles and a year later the same lifter #4 intake has failed again. This time it will move for 3 cam revolutions and then the rocker stops moving. Am i missing something here? Has anyone seen this before? What did you find? What should i check before i send this back out again?

The afm lifters are pretty much crap. Even if not being activated, they are still failure prone. Post covid, The regular oem lifters aren’t much better either.

Got the head off and found a chewed up roller. I'm guessing that the lifter collapsed and bounced around on the cam

I would guess the cam is chewed(screwed) too. Teardown time....
