Default Torque converter bolt?

So my friend just put a newly balanced stock flexplate in the car tonight. Good news is that it fixed the vibration. When he was on his way back from a test drive, he rolled Ito first pretty good and heard a thump sound. When he got it on the lift he found a sheared off torque converter to flex plate bolt. What could have caused this? They were all tight with red locktite on it.... the only thing I can think of is that he over torqued it.... he used an impact to tighten it. Anything else that could wheat a single bolt off??

How did he get an impact wrench in there to tighten the converter bolt?

Not possible to use an impact wrench on a torque converter bolt. Did you mean flywheel to crank bolt?

And I would immediately suspect user error, whether the wrong bolt was used, installed from the wrong side, etc. Who uses an impact gun to tighten bolts like that?
