Weird T56 Problem?

Weird T56 Problem? Sponsors, or anybody else for that matter, have you seen this issue?
Yesterday I broke down my T56 out of my '04 GTO. I was having issues shifting from 1st to 2nd.
Car JUST rolled over 11k miles. The clutch that was having issues was put in around 6k miles. The clutch was never fully disengaging.
I thought the blocker rings would be worn down, but they actually looked pretty good, except for the laminated friction material had FALLEN OFF, on 1'st and 2'nd gear????
Is this a common problem? My trans guy said he's never seen that with a manual trans before.
