Default Car still doesnt shift into 3rd/D

Had performabuilt rebuild the tranny after I lost all gears, after i get it back i still dont have 3rd or 4th gear in my 4L60E. Next thing I am checking is my tune. Can someone look at my tune and tell me if there is anything wrong with it, I am going to do a scan today as well. I have tried everything, get up to 40 mph at 5500 rpm and it still didnt shift. thanks for all the help.

your trans setup is all stock except for tq reduction/abuse mode, so assuming that your tire height isn't too far out, I'd say it's something physical

I'd be more worried about how dead-lean the car is under throttle. With 02 mV in the single and double digits, your are leaner than 15:1 at WOT. If you look at your logs, it is WAY under reporting cylinder fill, so don't jump and fatten it up just yet. You can see it reflected in your injector time firstly. I expect that 28 deg across the board will be too aggressive in many places, but I don't know your SCR, so I couldn't say for sure. The KR at WOT cannot be looked at as a timing issue with the car being that lean. I see that you have the MAF fail freq at 0 Hz and you have the DTCs altered, but you have it set to fail on second error, not first. It looks like the car was still trying to reference the MAF; it wasn't really in SD. Change the DTC to fail on first and go get another log. Best of luck.
