HELP Code 171

It reads Bank 1 sensor 1 is Lean
My mods are Ported MAF Screen removed, stock injectors custom handheld tune, Pacesetter LT Custom catted Y 222/228/115 cam stock heads. This code does not come on all the time but occasionally. I took cat to dealer on tech 2 and he said O2's look good it could be your fuel or ethanol they have in fuel or carbon build up. Does anyone have this same problem. My AFR from Dyno is 12.5-12.8 I dont know what else to change or do?
The car runs good as far as performance goes, Just the SES light come on and gives this code. All help appreciated!!!

Everything has been tuned. I dont hear any leaks from exhaust when engine is running. I did check for a vacume leak where the blk rubber hose reaches the manifold on passenger side. There was some light black soot on my fingers, had faint smell of gas. So I put a zip tie to it. I dont know if this was the culprit, but I hope so.
I have stock MAF (ported) I check everything I could as far as hoses. With exception of what I just explained above.

currently dealing with the same code. From searching on here, it looks like its:
Vacuum leak after the MAF
Aftermarket MAF
Dirty MAF
o2 sensor
Still haven't tracked it down though...

I wish I had known this before taking car into dealer on a TECH 2 machine. Could've saved $50.00 bucks. Hope this helps.
