Is there a list for a mail order tune?

I need to get my pcm tuned for my 5.3 and its parts and sensors i deleted. Not sure what all i have to state that i have and what sensors are required. Is there like a check list or form i can fill out to make sure i dont forget anything that way i dont have to do it twice? also is it recommended to get a mail order tune on a 226/226 588/588 comp cam or is it best to do it on a dyno? to make sure i get the max ponies out of my motor?

tuning

A dyno tune or a street tune is the best way to go but Steve will hook you up good with a mail order you will not be disapointed

so a gen 3 truck mail order tune is 225 plus shipping?
