startup/idle tuning after cam
is a good guide. If it's hot starts it's probably going into closed loops and the LTFT tables is not yet established and can make idling a bit of a nightmare. If it doesn't resolve after 30 minutes of varied driving it could be something else. I am not an expert, but I have run into that before. Really the MAF should be able to even that out though.
You don't even know if a startup problem is even a startup problem until the MAF, VE, idle airflow, and spark is tuned. You are most likely trying to bandaid bad MAF/VE/idle airflow tables by just adding in startup airflow. Look at tuning as a large process. Read through the info in the link that Zinner posted and spend some time reading over at the HPTuner's forum.
