Hesitation and Sluggishness

Hesitation and Sluggishness I've had my car for about 5 months now and it's had the same issue the entire time. Looking for some ideas.
At 2500 rpm, under load and no matter the gear and throttle position (most noticeable at low throttle), there is very obvious hesitation. Also...at wide open throttle around 5000rpm towards red-line, the car falls on it's face.
Idle to 2500 rpm = solid pull
2500-3000 = hesitation but runs
3000-5000 = still pulling but not near as strong
5000-redline = falls on it's face...
The only code the car is throwing is a catalyst efficiency below threshold.
I'm going to be doing some data logging to see if it's pulling timing, which I believe it is. Plugs and wires are done. Going to replace the MAF as one in the car is very dirty and one of the "connections" (unsure of what they are called) looks burnt. O2 sensors have been checked...3 were fine, one was bad...bad one replaced. Good fuel has been used since purchasing.
Long-tubes and y-pipe will be taken care of sometime this summer with a tune to follow along shortly so that catalyst code will disappear...along with a few other bolt ons to make the tune worth while.
Any thoughts as to what may be the issue? Contemplating picking up bigger replacement injectors to help support the H/C I want to do.
