Need Tune File for LQ4 with Manual

So long story short I am trying to get my conversion on the road and it appears that my segment swapped tune is not working, does any one have a LQ4 manual tune I can use with EFI LIVE? It is a 2003 LQ4 (blue/green) out of an Express. I have found several vins with factory 2003 LQ4 and manual online but I have no way of getting the factory tune from them (see below). Current OS is 12593058.
