Help with po121 and po300 ......

Help with po121 and po300 ...... ok I've tried everything now I keep getting the flashing SES light with codes po121 and po300 twice. I've replaced the plugs, plug wires, pcv(hose had hole in it). And just letting it idle it will be fine and then after a few minutes it will start flashing .... I have a loss of power down low when I floor it the car will stay steady for a second or 2 then haul the mail like no ones business. Especially from a dig it will hold it for a second or 2 and then wake up. Any clue what it could be ?
