Default Hot bank cold bank running issue

Hi guys, posting because I’m fresh out of ideas on a running issue that is driving me insane.
I have a conversion here is Aus running an L77 6.0 with an e38 ecu. Vats removed reflashed to a manual by the tuner because the motor was originally an auto.
My issue is that I have one bank running hot and one stone cold. The hot bank is my drivers side (US passenger). After hours and hours and hours spent checking and testing I have worked out that the injectors are firing in a wildly random order and I suspect only a couple of cylinders are actually combusting hence the heat and lack of heat elsewhere. I know this because when using the scan tool with the noid light on injectors I turn off certain cylinders and the light doesn’t turn off. The motor has had cam change including pushrods, springs etc.

Things I have checked:
Compression - good
Leak down test - good
Intake smoke test - good
Injectors all pulsing
Great spark.
Coils all firing
Replaced CMP
Checked all springs and valves for opening and closing.
Tested all wiring back to ecu plug to confirm correct colour, pin location, continuity etc
Checked all grounding
MAF reads 95kpa atmosphere, 76-78kpa while running
Great fuel pressure

I am at an absolute loss what the issue might be??
Fire off your questions, what else should I check??

What altitude are you at?
What cam?
Inj harness plugs crossed?

Yes MAP sorry not MAF
400m above sea level
Cam is the VCM11
The harness plugs aren’t crossed I have confirmed each one by the colour of the trigger wire from the pin out schedule back to the ecu plug.

Update:

Today I repinned my ecu plug to the order that the injectors are firing in. The motor ran exactly the same, hot and cold, effectively no change.
Im now almost certain the motor doesn’t know where it is in the rotation cycle. It has no reference.

Im wondering if the tuner has programmed in the wrong tooth count crank reluctor wheel???
Has anyone ever had that experience?

You can't change that in a stock ECU. Only aftermarket.
