Safe, effective way to raise the idle?

Safe, effective way to raise the idle?
My idle is low enough that I have a pronounced squeak/rattle in the exhaust in gear w/ the A/C on. Without the A/C it is occasional but not too bad. In park its not there. Its totally RPM driven...especially since I have a vig3200 w/ lowered my idle.
Now, the question is how to I raise it free and easy. I have done that idle relearn crap, and played with the idle set screw. If I raise it at all it surges.
So...drill a hole in the TB? What is too big? if I go too big will it affect the car? Will the PCM relearn the low idle on its own?
What else could I do?
